Background: Issues concerning legislation and regulation with respect to the role of nurses in euthanasia and physician-assisted suicide gave the Minister for Health reason to commission a study of the role of nurses in medical end-of-life decisions in hospitals, home care and nursing homes.Aim: This paper reports the findings of a study of the role of nurses in euthanasia and physician-assisted suicide, conducted as part of a study of the role of nurses in medical end-of-life decisions. The findings for hospitals, (...) home care and nursing homes are described and compared.Method: A questionnaire was sent to 1509 nurses, employed in 73 hospitals, 55 home care organisations and 63 nursing homes. 1179 responses were suitable for analysis. The questionnaire was pilot-tested among 106 nurses, with a response rate of 85%.Results: In 37.0% of cases, the nurse was the first person with whom patients discussed their request for euthanasia or physician-assisted suicide. Consultation between physicians and nurses during the decision-making process took place quite often in hospitals and nursing homes and less frequently in home care situations . In some cases , nurses administered the euthanatics.Conclusions: The results show substantial differences between the intramural sector and the extramural sector , which are probably linked to the organisational structure of the institutions. Consultation between physicians and nurses during the decision-making process needs improvement, particularly in home care. Some nurses had administered euthanatics, although this task is by law exclusively reserved to physicians. (shrink)

BackgroundPreprint usage is growing rapidly in the life sciences; however, questions remain on the relative quality of preprints when compared to published articles. An objective dimension of quality that is readily measurable is completeness of reporting, as transparency can improve the reader’s ability to independently interpret data and reproduce findings.MethodsIn this observational study, we initially compared independent samples of articles published in bioRxiv and in PubMed-indexed journals in 2016 using a quality of reporting questionnaire. After that, we performed paired comparisons (...) between preprints from bioRxiv to their own peer-reviewed versions in journals.ResultsPeer-reviewed articles had, on average, higher quality of reporting than preprints, although the difference was small, with absolute differences of 5.0% [95% CI 1.4, 8.6] and 4.7% [95% CI 2.4, 7.0] of reported items in the independent samples and paired sample comparison, respectively. There were larger differences favoring peer-reviewed articles in subjective ratings of how clearly titles and abstracts presented the main findings and how easy it was to locate relevant reporting information. Changes in reporting from preprints to peer-reviewed versions did not correlate with the impact factor of the publication venue or with the time lag from bioRxiv to journal publication.ConclusionsOur results suggest that, on average, publication in a peer-reviewed journal is associated with improvement in quality of reporting. They also show that quality of reporting in preprints in the life sciences is within a similar range as that of peer-reviewed articles, albeit slightly lower on average, supporting the idea that preprints should be considered valid scientific contributions. Background In Belgium, termination of pregnancy after the first trimester is exclusively allowed on medical grounds. When faced with fetal or maternal health complications during pregnancy, patients typically turn to obstetricians for guidance on the diagnosis, prognosis, and available options. Patients’ decisions and their actual access to termination of pregnancy can be profoundly influenced by the quality of this counselling and the willingness of professionals to present termination as an acceptable option. This paper aims to explore the factors influencing obstetricians’ (...) acceptance of TOP requests after the first trimester of pregnancy. We subsequently analyze these acceptance dynamics from a multidisciplinary angle, incorporating ethical perspectives and a socio-legal exploration into how the interviewed health professionals experience, interpret, and apply the law. Methods We conducted an interview study with 23 hospital obstetricians who had prior experience with termination of pregnancy decision-making beyond the first trimester in Flanders, Belgium. Interviews, on average, lasted 1h30 and followed a semi-structured format guided by a topic guide. The transcripts were coded with NVivo software and subsequently thematically analyzed by a multidisciplinary research team to provide a comprehensive understanding of obstetricians’ acceptance of termination of pregnancy after the first trimester. Results Obstetricians’ acceptance of termination of pregnancy after the first trimester mainly depends on the presence of compelling clinical factors. Secondary factors, including patient/couple preferences, institutional and collegial processes, timing and viability, technical considerations, obstetricians’ ethical and professional values, the wider background of the patient/couple, and perception of alternatives, could sway decisions in the absence of compelling clinical factors. Conclusions Secondary factors help sway obstetricians’ decisions in favor of or against termination of pregnancy after 12 weeks when a request is characterized by inconclusive clinical factors. The multifactorial acceptance dynamics of obstetricians illustrate the limits of a strong emphasis on fetal interest argumentation. Moreover, they exhibit a degree of divergence and complexity absent from the Belgian Abortion Law. The presented typology of factors could stimulate and guide debates on legal reform and the importance that should be attributed to various factors in professional decision-making on termination of pregnancy. Health technology assessment (HTA) is often biased in the sense that it neglects relevant perspectives on the technology in question. To incorporate different perspectives in HTA, we should pursue agreement about what are relevant, plausible, and feasible research questions; interactive technology assessment (iTA) might be suitable for this goal. In this way a kind of procedural ethics is established. Currently, ethics too often is focussed on the application of general principles, which leaves a lot of confusion as to what really (...) is the matter in specific cases; in an iTA clashes of values should not be approached by use of such ethics. Instead, casuistry, as a tool used within the framework of iTA, should help to articulate and clarify what is the matter, as to make room for explication and consensus building. (shrink)

Part of the generalizability issues that haunt controlled lab experiment designs in psychology, and more particularly in psycholinguistics, can be alleviated by adopting corpus linguistic methods. These work with natural data. This advantage comes at a cost: in corpus studies, lexemes and language users can show different kinds of skew. We discuss a number of solutions to bolster the control.

The existence of the German cultural community was guaranteed by the articles 3ter and 59ter of the Constitution and its organisation was institutionalized by the Law of July 10th, 1973. In 1980 the notion «German cultural community» in art. 3ter of the Constitution was replaced by the notion «German-speaking community».The reform of the statute of the German-speaking community has to be executed within the following limits : 1° an executive power that is accountable to the council can only be granted (...) after revision of art. 59ter of the Constitution; 2° there seems no harm in granting power to modify laws and to legislate with exclusion of the national legislator to the council, as long as the exercise of this power is submitted to, judicial review ; 3° the enlargement of the council's competence to the «personalized matters» does not require the revision of art. 59ter. The enlargement of the council's competence to some «regional affairs» is possible without revision of the Constitution, as long as the council does not receive the same competence as the Flemish, Wallon and Brussels region; 4° it is possible to grant the German-speaking community theyield of a part of some taxes. Dutch, like other Germanic languages, disposes of two strategies to express past tense: the strong inflection (e.g., rijden – reed ‘drive – drove’) and the weak inflection (spelen – speelde ‘play – played’). This distinction is for the most part lexically determined in that each verb occurs in one of the two inflections. Diachronically the system is in flux though, with the resilience of some verbs being mainly driven by frequency. Synchronically this might result in variable verbs (e.g., schuilen – (...) schuilde/school ‘hide – hid’ or raden – raadde/ried ‘guess – guessed’). This diachronic (1300–2000) corpus study shows that this variation is not haphazard, but that semantic factors are at play. We see two such effects. First of all, synchronically, the variation is exapted in an iconic manner to express aspect: durative meanings tend to be expressed by longer verb forms and punctual meanings tend to be expressed by shorter verb forms. Secondly, we see that metaphorical meanings come to be associated within obsolescent inflectional forms, as predicted by Kuryłowicz’s “fourth law of analogy”. (shrink)

High-quality doctor-patient communication in end-of-life care results in better quality of life for patients. In linguistically and culturally diverse societies, language discordant consultations become daily practice, leading to difficulties in eliciting patient preferences toward end-of-life care. Although family members invariably act as informal interpreters, this may cause some ethical dilemmas. We present a case of a palliative patient whose son acted as an interpreter. This case generated a triple- layered ethical dilemma: how to safeguard patient autonomy against paternalistic interventions by (...) family members, how to respect the relational context in which patient autonomy can be realized, and how to respect the ethno-cultural values of the patient and his family. These issues are being discussed and reflected upon within the framework shared decision making involving informal- and professional interpreters. The complementary use of professional interpreters next to family members acting as informal interpreters is recommended. (shrink)

We investigate empirically whether framing in general, and the use of Simplifying Models as a framing tool in particular, has an effect on the way topics are cognitively construed. Existing studies on framing in linguistics have either been theoretical or descriptive. Going beyond such methodologically simple approaches, we use a more rigid test design involving the use of a control group, the construction of test conditions in which different Simplifying Models constitute the major source of variation, the inclusion of independent (...) variables like age and prior knowledge of the subjects, and the use of linear and logistic regression analysis. Our results show that our more rigid methodological approach yields a more reliable image of the effect of Simplifying Models on the way in which people deal with information on a complex topic like sustainable food production. Fleshing out these effects further may in time lead to a better informed construction of communication on complex social topics. (shrink)

On the basis of synchronic English language material, Bolinger (Degree Words, Mouton, 1972) has put forward the hypothesis that intensifying meanings or “degree words” often develop from identifying expressions. This paper will empirically test Bolinger's hypothesis by means of in-depth diachronic study of the development of such—one of Bolinger's central examples—and of its Dutch cognate zulk in historical text corpora. To this aim, a detailed cognitive-functional account will first be provided of the (differences between the) identifying and intensifying uses of (...) such and zulk, with attention for diachronic changes affecting the syntax and semantics of these uses, cross-linguistically as well as language-specifically. It will be shown that, as predicted by Bolinger (Degree Words, Mouton, 1972), the proportion of identifying uses decreases over time in favor of the intensifying uses both in English and Dutch. The comparison between such and zulk will, however, show that, despite the close relation between these two languages, the development does not run strictly parallel in English and Dutch, thus endorsing a view that language change does not necessarily follow predetermined pathways. We will argue that minute differences in the syntax of such and zulk steer the diachronic course these elements follow. Finally, Bolinger's shift from identification to intensification will be discussed in terms of its relation to existing (inter)subjectification hypotheses. (shrink)

Van Peursen’s book is an authorized translation and is published under the auspices of the Duquesne Philosophical Series. The purpose of the work is to effect a rapprochement between two of today’s most notable approaches to philosophy: phenomenology and linguistic analysis. These respective philosophical methods are frequently looked upon as two of the conspicuous polar trends within contemporary philosophy, and the author arranges their confrontation in such a way that their most fundamental convergencies and divergencies are revealed. The original phase (...) of phenomenology, that of Edmund Husserl, and the earlier phase of logical positivism as represented by Carnap and Schlick, are described by focusing on their treatment of the problem of meaning. A further comparison is made between the respective ways phenomenological reduction and analytical reduction treat the question of factual existence. Both philosophical methods are presented clearly and sympathetically in their historical depth and with emphasis on their methodological aspects. The author then makes a provisional confrontation between them which centers on the critical problem of defining the appropriate relation between operational and intuitive meaning, the truths of reason and the truths of fact. These two methods of philosophy stem from essentially different attitudes whose significance transcends the limits of philosophy, and one of Van Peursen’s purposes is to explain both schools of thought to a wider public. He then traces the further development of phenomenology through the later Husserl and Merleau-Ponty, while concentrating on the phenomenology of the ego and of the Life-World. This analysis is followed by a description of the movement from logical positivism to analytical philosophy as it was shaped by the linguistic orientation of Wittgenstein and Ryle. The last chapter offers a final confrontation between the transcendental and the linguistic approaches. (shrink)

Human cognition is unique in the way in which it relies on combinatorial (or compositional) structures. Language provides ample evidence for the existence of combinatorial structures, but they can also be found in visual cognition. To understand the neural basis of human cognition, it is therefore essential to understand how combinatorial structures can be instantiated in neural terms. In his recent book on the foundations of language, Jackendoff described four fundamental problems for a neural instantiation of combinatorial structures: the massiveness (...) of the binding problem, the problem of 2, the problem of variables, and the transformation of combinatorial structures from working memory to long-term memory. This paper aims to show that these problems can be solved by means of neural “blackboard” architectures. For this purpose, a neural blackboard architecture for sentence structure is presented. In this architecture, neural structures that encode for words are temporarily bound in a manner that preserves the structure of the sentence. It is shown that the architecture solves the four problems presented by Jackendoff. The ability of the architecture to instantiate sentence structures is illustrated with examples of sentence complexity observed in human language performance. Similarities exist between the architecture for sentence structure and blackboard architectures for combinatorial structures in visual cognition, derived from the structure of the visual cortex. These architectures are briefly discussed, together with an example of a combinatorial structure in which the blackboard architectures for language and vision are combined. In this way, the architecture for language is grounded in perception. Perspectives and potential developments of the architectures are discussed. Cognitive agents are dynamical systems but not quantitative dynamical systems. Quantitative systems are forms of analogue computation, which is physically too unreliable as a basis for cognition. Instead, cognitive agents are dynamical systems that implement discrete forms of computation. Only such a synthesis of discrete computation and dynamical systems can provide the mathematical basis for modeling cognitive behavior.

We discuss a visual blackboard architecture that could be involved in imagery. In this architecture, networks that process identity information interact with networks that process location information, in a manner that produces structural (compositional) forms of representation. Architectures of this kind can be identified in the visual cortex, but perhaps also in prefrontal cortex areas related with working memory.
